To submit a Public Service Announcement, you must complete the following three steps.
First, create an account or access an existing account.
Second, register online and complete the required paperwork. Please submit the following forms online:

Registration form;

Certification and Release form; and

Personal Release form, which must also be filled out by all actors/models and uploaded with your entry.
For group entries, each group member must fill out a separate Registration Form as well as a Certification & Release form. Forms may also be found in the Contestant section of the website.
Third and final, upload your video or print PSA.
Once you have successfully uploaded your PSA, you will be automatically directed to a page confirming CIDI's receipt of your entry and will be officially entered into PSAid 2012. If you do not, please contact the contest coordinator.
